Home Care and Community-Based Services

 

Personal care, housework, meal preparation, grocery shopping, and laundry are some of the basic, in-home services we can help you access.  You may also attend a day care program, receive assistance with transportation, or access overnight respite care in a nursing facility or assisted living center.   Bristol case managers help elders remain as independent as possible in their communities by assessing the needs of each individual who requests assistance and coordinating the appropriate services. For elders who are eligible, Bristol’s Home Care and Respite Programs offer access to the many community-based services.  For others, Bristol case managers help access assistance privately.  Either way, an assessment by one of Bristol’s knowledgeable case managers is the place to begin.  A telephone call, visit, or e-mail will connect you to a wealth of information and resources.
