The successful candidate will work with elders enrolled in a Senior Care Options (SCO) plan which provides health and social support services normally paid through Medicare and MassHealth.  The GSSC will conduct in home assessments, reassessments, ongoing case management, and service planning in collaboration with Care Partners from the SCO health plans.  The GSSC is responsible for monitoring the effectiveness of service plans and promoting consumer independence and choice.  Maintaining documentation in electronic consumer records according to established procedures is a must.  Bachelor’s degree in Social Work, Human Services, or a related field; valid driver’s license; and reliable transportation required.  Experience working with disabled adults and/or elders preferred.
