Home Care Case Assistant
The successful candidate will possess clerical experience as well as customer service experience, to relate to consumers of all different personalities. The Home Care Case Assistant will conduct daily activities related to the Home Care Case Management Department. The Case Managers coordinate in-home services for consumers who are 60 years of age and older and work with informal supports as well as a wide variety of community providers. The Home Care Case Assistant will provide support to the Case Managers by screening, routing, and responding to incoming telephone calls from consumers/providers; conducting follow-up assistance and support work on behalf of the elder; carrying out a variety of office support tasks for team members such as mailings, preparing documents, enteringing consumer information into the database; completing and maintaining reports and spreadsheets for the department. The Home Care Case Assistant will also complete some field work to assist consumers with paperwork related to the Home Care Program.
High school diploma or equivalent, associate degree in a Human Service field preferred, one-year experience with telephone and general office experience required. Customer service skills and effective communication skills a must. Ability to work in a fast-paced, high volume environment. 35-hour work week, 9:00 a.m. – 5:00 p.m., Monday through Friday. Fluency in English and Portuguese or English and Spanish preferred.
