Home Care Case Manager
As a member of an interdisciplinary team focused upon supporting elders to remain independent in their homes, the successful candidate will provide a client-centered approach to assessing consumer needs, developing service plans, and maintaining consumer records.
The case manager assesses consumers in their homes to determine eligibility for home care program services, develops service plans in conjunction with consumers and their support network, and monitors consumers’ well-being and the effectiveness of service plans. The case manager is also responsible for maintaining information relating to each case in accordance with the MA Executive Office of Elder Affairs’ regulatory guidelines.
- Bachelor’s degree in a social service related field
- Valid driver’s license
- Reliable transportation
