Supportive Housing Case Manager

Bristol Elder Services is seeking a dynamic candidate with enthusiasm and an ability to connect with residents living in a Supportive Housing environment in the Fall River area. The applicant will be responsible for coordinating and conducting educational, social and recreational activities at the housing site.  The applicant will be available on-site to provide office hours for residents and maintain a regular visible presence at the supportive housing site.  The applicant will also be working closely with the housing authority and the vendor servicing residents in the building.  This position will require collaboration with another Supportive Housing Case Manager to coordinate the program and address the needs of the residents.  The applicant will also be responsible for a small caseload of clients living in the community.  As part of that role, the applicant will conduct in-home needs assessments, coordinate services, and provide advocacy and on-going case management.


  • Bachelor’s degree
  • At least six months’ elder care experience
  • Valid driver’s license
  • Reliable transportation
  • Experience coordinating social and wellness programming a plus
